How We Compiled This List
A list of nursing homes considered “special focus facilities” is published on the Centers for Medicare & Medicaid Services (CMS) website. These facilities are the long-term care facilities and nursing homes with serious quality issues or facilities included in a special program to improve their quality of care.
That list, in addition to Medicare’s Nursing Home Compare tool, helped us create this list and better understand which nursing homes need improvement.
The Lowest-Rated Nursing Homes in the Boston Area
The Oxford Rehabilitation & Health Care Center in Haverhill, MA, made CMS’s SFF list of nursing homes that were previously considered a special focus facility, yet made no improvements.
Nine other facilities in the Boston area received only one star in Medicare’s Nursing Home Compare tool, making them the lowest-rated facilities in the region. Those facilities include:

- Medford Rehabilitation and Nursing Center (Medford, MA)
- Vero Health & Rehab of Revere (Revere, MA)
- Mattapan Health & Rehabilitation Center (Mattapan, MA)
- Stonehedge Health Care Center (West Roxbury, MA)
- Melrose Healthcare (Melrose, MA)
- Quincy Health and Rehabilitation Center LLC (Quincy, MA)
- Brush Hill Care Center (Milton, MA)
- Wingate at Needham (Needham, MA)

Though it’s not a foregone conclusion your loved one is being mistreated in these one-star facilities, these lists provide valuable insights for those concerned about the quality of care offered in nursing homes.
It’s also true that if you’re concerned about your loved one’s care, you shouldn’t assume that those concerns are unfounded if you discover the facility doesn’t make our list. It’s possible, even in facilities that fare well in rating systems, that residents might be subjected to abuse or neglect.
