If you’ve ever asked yourself whether some nursing homes offer lesser quality care than others, then you should know that there are official ratings systems that help people address this exact question.
Medicare rates long-term care facilities and makes its ratings public. It’s wise for anyone with a loved one in a nursing home to find out the quality of care their family member receives—or to do research before you sign the paperwork and move your loved one into their new home. To help you determine the nursing homes in the Boston area with the worst ratings, we’ve compiled a list below.
How We Compiled This List
A list of nursing homes considered “special focus facilities” is published on the Centers for Medicare & Medicaid Services (CMS) website. These facilities are the long-term care facilities and nursing homes with serious quality issues or facilities included in a special program to improve their quality of care.
That list, in addition to Medicare’s Nursing Home Compare tool, helped us create this list and better understand which nursing homes need improvement.

The Lowest-Rated Nursing Homes in the Boston Area
The Oxford Rehabilitation & Health Care Center in Haverhill, MA, made CMS’s SFF list of nursing homes that were previously considered a special focus facility, yet made no improvements.
Nine other facilities in the Boston area received only one star in Medicare’s Nursing Home Compare tool, making them the lowest-rated facilities in the region. Those facilities include:
- The Rehabilitation & Nursing Center at Everett (Everett, MA)
- Medford Rehabilitation and Nursing Center (Medford, MA)
- Vero Health & Rehab of Revere (Revere, MA)
- Mattapan Health & Rehabilitation Center (Mattapan, MA)
- Stonehedge Health Care Center (West Roxbury, MA)
- Melrose Healthcare (Melrose, MA)
- Quincy Health and Rehabilitation Center LLC (Quincy, MA)
- Brush Hill Care Center (Milton, MA)
- Wingate at Needham (Needham, MA)
We strongly suggest you review the Nursing Home Compare tool to learn more about a facility that you’re considering placing your loved in or that your loved one currently resides in.
Though it’s not a foregone conclusion your loved one is being mistreated in these one-star facilities, these lists provide valuable insights for those concerned about the quality of care offered in nursing homes.
It’s also true that if you’re concerned about your loved one’s care, you shouldn’t assume that those concerns are unfounded if you discover the facility doesn’t make our list. It’s possible, even in facilities that fare well in rating systems, that residents might be subjected to abuse or neglect.
When Your Loved One Is Mistreated, You Have Legal Options
Nursing homes have an obligation to keep residents safe. This means not only providing sufficient care, but also ensuring that residents aren’t abused or neglected. When someone is seriously injured in a nursing home, it’s likely the injury stemmed from inadequate care, which means that the nursing home can potentially be held accountable for the costs of that injury.
When your loved one has been seriously injured in a nursing home, you have a right to pursue legal options to get them the care they deserve and receive compensation for the costs of your loved one’s injury.
